INSURANCE AND POLICIES
Insurance: We are members of Delta Dental, Guardian, Principal Insurance, Ameritas, Met Life, Aetna, GEHA, Connection, Humana, and Dental Source PPO. We are not members of any discount plans or Medicaid. We will file the insurance for patients; co-pays are due at the time of service. We will file for Blue Cross Blue shield and Great West insured patients even though we are not members of their groups. We do not take installment payments on services.
Scheduling Policies: 24 hours notice for canceling an appointment is required, otherwise a $50.00 fee will be assessed and will need to be paid prior to being re-appointed. Of course if the reason for canceling is a family emergency or illness, those things which can not be planned for, no fee will be charged. If a new patient misses their first appointment with less than 24 hours notice they will not be re-appointed, and if a patient chronically misses appointments we may dismiss them from treatment. As much notice as possible when canceling appointments is greatly appreciated as it allows us to better serve the community by seeing emergency patients during those time slots.
